From what I can tell from my exeprience so far to get the purchase option to pop up again  either click on training while you have mission on going or vice versa , of course the same goes for special missions too

Does anyone know if the 99 donut purchase is a one off when the free time ends, or is it offered forever?

While running a mission, click on the training tab (or the other way around), once your initial trial ends, and you will be prompted to click either a weeks worth for 9 donuts or permanently for 99...
